Fiorentina were taken to extra time by Jagiellonia Bialystok before a Nicolo Fagioli cracker and Giovanni Fabbian's deflected goal eventually secured their passage to the UEFA Conference League last 16.
A Bartosz Mazurek hat trick for the visitors saw the Viola surrender their three-goal advantage from last week's first leg in Poland within the first hour of play so, with the sides level at 3-3 on aggregate after 90 minutes, another 30 were needed.
In the second half of extra time, Fagioli fired home from the edge of the box to put Fiorentina ahead, then Moise Kean's shot was turned in by Fabbian to finally give Paolo Vanoli's side a cushion.
With two minutes left on the clock Jesus Imaz pulled one back to give Jagiellonia a 4-2 lead on the night but the Viola held on to advance 5-4 on aggregate.
